SELF is a non-profit organisation, providing informed advice & support to help clients manage their health. Our service is aligned with the Government's strategy for well-being and prevention. We aim to improve the health of the community by sharing knowledge through health education and awareness to support well-being.

We help people to help themselves, providing them with the knowledge to understand their health and wellbeing, and supporting them to build confidence in self-care and self-management.

Our aim

Most individuals need a different kinds of support to motivate them to make lifestyle changes, to care for their long term condition. Patient / Professional Partnership require collaborative care and self-management education. This is based on a person-centred approach, which implies that the individual’s perspectives on their condition - their goals, expectations , and needs - must become the focus of their treatment goals and care management activities.


Since 95% of the care is undertaken by individuals receiving the care, then it is prudent that they acquire the knowledge and skills to effectively manage their own care and for such individuals to be recognised and be respected as part of the team serving a common purpose. Having worked within the Brent community and with the multi-disciplinary team  ( ie. NHS,Social Services and other Health organisations) regarding long term conditions, then it is reasonable that individuals are encouraged to learn from healthcare professionals to become more proficient at undertaking their own self-care management for their condition on a daily basis. SELF will share such knowledge and experience with individuals to enable their empowerment to take charge of their own well-being. 


We aim to improve the well-being of the community by sharing knowledge through health education and building awareness to support well-being. Individuals will learn from our Healthcare Professionals how to become better self-care managers of their health and any long-term conditions.
